After School Program Registration

I authorize my child to participate in UCMP’s after-school program. I understand that I must ensure that my child follows the rules and agree to make sure he/she will be on time for his/her lessons. I will also cooperate with UCMP’s policy on attendance and behavior in this after-school program and have talked to my child about what is expected of him/her.*
